Apply for Medical Certificate
PROCEDURE OF OBTAINING A MEDICAL CERTIFICATE
STEP 1: RECEPTION
- Reception on 3rd floor City Hall – Public Health Department customer care desk
- Payment voucher is raised for the client to go and make payments at the cash office
- Customer is required to provide his/her:
- Name
- ID number
-Name of food plant/ Where you work
- Plot number
- Road on which plot is on - Customer obtains an official receipt for the service
- Customer makes photocopy of receipt and lab form
- Customer goes to report to the nearest Laboratory for examination (STC casino, Ngaira, Lang’ata, Mathare North, Westlands H/C & Kangemi)
- Customer is required to provide at the lab:
- Full names
- Identity Card number
- Name of food plant
- Plot number- Road/street
STEP 2: LABORATORY EXAMINATION & ANALYSIS
- Samples from food handlers taken & analyzed at the laboratory
- Sick person to be notified for treatment & re-examination in the facility (Period depends on the disease)
- Healthy person (Results to be released to City Hall for Medical certificate processing
STEP 3: PROCESSING OF CERTIFICATES
- Preparation of certificate after the laboratory forms come from the labs (First In First Out)
- Entering of clients names, places of work, plot no’s and road into the computer and then printing of the Medical certificate based on information contained in lab forms
- Signing of Medical certificates by an assigned clinician
- Stamping & Sealing of certificates
- Dispatch back to clients
- The certificate is valid for 6 months and takes 2 days to be processed

600/- ksh per person
