Apply for Medical Certificate

PROCEDURE OF OBTAINING A MEDICAL CERTIFICATE
STEP 1: RECEPTION

  1. Reception on 3rd floor City Hall – Public Health Department customer care desk
  2. Payment voucher is raised for the client to go and make payments at the cash office
  3. Customer is required to provide his/her:
    - Name
    - ID number
    -Name of food plant/ Where you work
    - Plot number
    - Road on which plot is on
  4. Customer obtains an official receipt for the service
  5. Customer makes photocopy of receipt and lab form
  6. Customer goes to report to the nearest Laboratory for examination (STC casino, Ngaira, Lang’ata, Mathare North, Westlands H/C & Kangemi)
  7. Customer is required to provide at the lab:
    - Full names
    - Identity Card number
    - Name of food plant
    - Plot number- Road/street


STEP 2: LABORATORY EXAMINATION & ANALYSIS

  1. Samples from food handlers taken & analyzed at the laboratory
  2. Sick person to be notified for treatment & re-examination in the facility (Period depends on the disease)
  3. Healthy person (Results to be released to City Hall for Medical certificate processing


STEP 3: PROCESSING OF CERTIFICATES

  1. Preparation of certificate after the laboratory forms come from the labs (First In First Out)
  2. Entering of clients names, places of work, plot no’s and road into the computer and then printing of the Medical certificate based on information contained in lab forms
  3. Signing of Medical certificates by an assigned clinician
  4. Stamping & Sealing of certificates
  5. Dispatch back to clients
  6. The certificate is valid for 6 months and takes 2 days to be processed
